Service Description

If you have had a New Patient Consultation and New Patient Follow Up, you are now considered an established patient. This is the appointment for existing patients to schedule if you need to continue a plan of care or discuss a new concern. You may have labs that need to be reviewed, or need to obtain a new lab order. We may be following up on treatment recommendations given at your previous visit and want to discuss improvements in your health or areas of concern that still need to be addressed. (If you have not had an appointment with us yet, please schedule a new patient consultation). *We do not bill insurance but you may use an HSA/FSA card or request a super bill for insurance reimbursement. Cancellation Policy

We ask that you please allow at least 48 business hours prior to your appointment to cancel or reschedule. If it is within 48 business hours of your appointment, there will be a $50 cancelation fee. For new patients there is a $50 non-refundable deposit due at the time of scheduling to secure the 90 minute time slot. We appreciate your understanding.
