Service Description

If you are an established patient and have less then 2 areas of concern to discuss, this is the appointment for you! The appointment is 15-30 minutes long, so if you have several areas to address or the concern is complex, please select the 60 minute follow-up appointment to allow enough time to address your concerns. (If you have not had an appointment at Balanced Health yet, please schedule a new patient consultation). *We do not bill insurance, but you may use an HSA/FSA card to pay for your appointment or request a super bill for insurance reimbursement. See Office FAQ.

Cancellation Policy

We ask that you please allow at least 48 business hours prior to your appointment to cancel or reschedule. If it is within 48 business hours of your appointment, there will be a $50 cancelation fee. For new patients there is a $50 non-refundable deposit due at the time of scheduling to secure the 90 minute time slot. We appreciate your understanding.
