Service Description

The Balanced Health Mini Package is for new patients wanting to establish care at Balanced Health Functional Medicine. This package includes a thorough evaluation of your past medical history, your current overall health as well as discussing any symptoms of concern you have. Symptoms may range from fatigue, stomach issues, autoimmunity, thyroid dysfunction, hormone imbalance, depression or anxiety, or anything else you may need help with. The package includes: - 3 Functional Medicine Visits: - One 90 minute initial visit (if labs need to be ordered, you will receive the lab order at this appointment) - One 60 minute follow up visit to review lab results and continue your plan of care - One 30 min visit to evaluate how the treatment plan is going. These appointments are generally spread over the course of 3 months but do not have an expiration date and can be used at any time. - An individualized treatment plan to include nutritional or lifestyle adjustments, supplement recommendations, prescription medications if needed and educational resources. - Access to your patient portal and direct communication with your provider in between appointments for urgent questions. - Insurance can be used for labs, prescriptions or other orders written by your provider. **Cost for this package is 3 monthly payments of $350. *You may use an HSA or request a super bill for insurance reimbursement. See Office FAQ. (A lab order will be provided and insurance can be used for labs or testing. If you do not have insurance, access to a low-cost cash pay option will be recommended). Cancellation Policy

As long as you allow at least 48 business hours prior to your appointment to cancel, you will have the option to reschedule or receive a full refund. If it is within 48 business hours of your appointment, there will be a $50 cancelation fee . If you are canceling your package, as long as you give 48 business hours notice or more prior to your appointment, you will receive a refund minus any visits you have completed. ($450 initial visit, $300/subsequent visits and additional cost if health/life coaching visits have been done).
