Office FAQ's

Can I use my health insurance for my office visit?

Visits are cash/credit card pay, or you may use your HSA/FSA card. At your request we can provide you with a superbill after your visit to present to your insurance company for reimbursement. (Please contact your insurance company to ensure this option is included in your insurance plan).

Can I use my health insurance for lab testing and prescription medications?

Yes! If there are any labs, specialty testing, imaging or prescriptions ordered at your visit - you can use your insurance.

Do I need to fill out the new patient paperwork at least 48 hours prior to my appointment?

It's important you fill out the new patient forms at least 48 hours prior to your first visit.  We set aside 90 minutes for your first visit to spend time on you and your health concerns.  We don't want your visit time to go to waste filling in forms.

What is the cancelation policy?

Please allow at least 48 business hours prior to your appointment to cancel or rescheduled.  If it is within 48 business hours of your appointment, there will be a $50 cancelation fee.

Is there a refund policy for the programs?

As long as you cancel with 48 business hours or more prior to your appointment, you will receive a refund minus any visits you have completed. ($450 initial visit, $350/subsequent visits and additional if health/life coaching visits have been done).
