Office FAQ's
Can I use my health insurance for my office visit?
Visits are cash/credit card pay, or you may use your HSA/FSA card. At your request we can provide you with a superbill after your visit to present to your insurance company for reimbursement. (Please contact your insurance company to ensure this option is included in your insurance plan). Medicare and Medicaid are not elligible for superbills.
Can I use my health insurance for lab testing and prescription medications?
Yes! If there are any labs, specialty testing, imaging or prescriptions ordered at your visit - you can use your insurance.
Do I need to fill out the new patient paperwork at least 48 hours prior to my appointment?
It's important you fill out the new patient forms at least 48 hours prior to your first visit. Alyssa also reviews all of the information you submit to prepare for your visit. Also, by having your forms filled out prior to your appointment, that allows all of your appointment time to be spent talking about you and your health instead of filling out forms. Thank you!
What is the cancelation policy?
Please allow at least 48 business hours prior to your appointment to cancel or rescheduled. If it is within 48 business hours of your appointment, there will be a $50 cancelation fee. For new patients or discovery visits, the $50 deposit paid at the time of scheduling is non-refundable.
Do you prescribe bioidentical hormones?
Yes! Bioidentical hormones are the only type of hormones Alyssa prescribes. Hormone replacement therapy can greatly improve the quality of life for both men and women. You may also be interested in discussing other options to help balance your hormones.
